You can download your group insurance claims report from the plan member site if your plan has this option.
Before you start
- If you don’t have this option, contact the person responsible for your group insurance plan.
- Note that the report is not an official tax document.
Log in to your account. Need help logging in?
Click on Claims history.
Click on Claims report for income tax purposes
In the Request a report section, choose the tax year from the dropdown menu and click on Request this report
The report will appear in the Consult my reports section.
If you request a new report for a later date in the same year, the previous report will be deleted.
Reports are saved on the site for 7 years.
